Job summary

We are looking for enthusiastic, dynamic, motivated registered Nurses to join the Rotherham Planned Community Nursing team. As a community Staff Nurse you will provide high quality patient- centred care, embodying the Trusts core values of ambitious, caring and together.

Main duties include, wound care, palliative care. IV drug administration, administration of injectable medication, diabetes management, pressure area care and a diverse range of other skills. You will support Patients to maintain independence and help avoid unnecessary hospital admissions. You will identify care needs, build strong therapeutic relationships with your Patients and work closely with other community services to provide the best possible health outcomes.

We have both full-time and part-time posts across 4 localities.

Due to the nature of the role it is essential that applicants are able to travel independently between TRFT sites and for patient visits in the Rotherham area and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.

Main duties of the job

You will be expected to visit Patients in their own home, undertaking a wide range of clinical interventions including wound care, medicine administration, palliative care, pressure area care, IV drug administration, diabetes management.
