Palliative Care Clinical Nurse Specialist

Job summary

We have an exciting opportunity for a Band 7, 25 Hours CNS to work within our highly skilled, motivated and dynamic team. We aim to make a difference and improve the experience of palliative care patients at various stages of their disease trajectory.The post will be based within the existing inpatient Palliative Care Team. We offer 7 day face to face support to ensure that people with specialist palliative care needs, will receive the best quality care.the post holder will act as a specialist resource Trust wide. Using Specialist Palliative skills and knowledge you will manage a caseload of patients and support their significant others and carers through effective, efficient evidence based high quality care.The delivery of education and training to multi-professional groups within the Trust is an integral part of the role.The post holder will also contribute to the teams' development, research and governance through the process of audit and bench marking.

Main duties of the job

To effectively assess, plan and provide specialist care and support to people affected by life limiting illnesses, and also to patients who are at the end of their lives.To participate as part of a multi-disciplinary, working collaboratively with colleagues, patients, carers, other health and social care professionals, statutory, voluntary and independent organisations to ensure optimum patient managementTo work within statutory legislation and local policies and procedures in relation to discharge planning and palliative care.To work autonomously and manage own case loadTo participate in providing a 7 day face to face serviceTo act as a resource, educator and advocate for patients, their carers and health care professionals.To participate in education initiatives to all members of staffTo participate in audit.
