Job summary
Internal acting up opportunity until 1st January 2027.
In collaboration and under the supervision of the District Nurse (DN), have clinical responsibility of a delegated caseload. Assist in the leadership, management and supervision of the Community Nursing Team
Main duties of the job
In collaboration and under the supervision of the DN, determine and prioritise the health needs of the Community Nursing caseload and contribute to the public health agenda for the local community, so resources can be targeted appropriately.
Work in partnership with members of the Health and Social Care Teams in the delivery of services to achieve collaborative and integrated working.
In collaboration with the DN and the wider integrated care team, manage and co-ordinate the assessment, planning, implementation and evaluation of care programmes, reflecting the needs of individual patients, carers and families.
Promote the principles of self-care, ownership of ones own health and patient empowerment.
In collaboration with the DN, lead and manage a caseload of the Community Nursing team, monitoring standards of care, delegating responsibilities and supervise team members as appropriate.
Due to the nature of the role it is essential that applicants are able to travel independently between TRFT sites and for patient visits in the Rotherham area and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.
