Job summary

Here on Ward B4 Care of the elderly 2y bed base Ward it is expected that the Discharge Coordinator works alongside our team of Nurses, Therapy, Pharmacy and Doctors to coordinate safe and timely discharges that come with a range of complexities requiring a multi-disciplinary approach.

Main duties of the job

The Discharge Coordinator will be expected to work as part of the Ward team under the direction of the Lead Nurse CRU with training and overview supported by Patient Flow Projects Matron. They will coordinate the discharge of Patients from the ward in line with the Trust standards and policies. The post holder will liaise and communicate effectively with relatives, community colleagues, discharge services, other allied health professionals and Medical/Nursing staff.
