Community Health Care Support Worker

Job summary

Various hours available

To work as part of the Planned District Nursing Teams, contributing to the care of housebound patients within their own homes. Undertaking delegated clinical duties, whilst working under the supervision of the senior nurse on duty. To provide basic wound care, administer medication and general holistic care to patients, ensuring their individual needs are met.

Main duties of the job

  • To provide clinical care to patients in their own homes as delegated by the senior community nurse within your own scope of practice.
  • To assist with prescribed medication administration to enable patients with long term conditions stay well at home.
  • Participate in the handover process to escalate any potential problems that may have occurred during patient visits
