We are seeking to appoint a Community Sister/ Charge Nurse for our Urgent Community Response Team.

This provides an exciting opportunity for those wishing to expand on their assessment and clinical skills within a community environment. The primary goal of our service is to prevent hospital admission by providing high quality care and treatment in the community setting, whilst maintaining the 2 hourly urgent care response target. You will work as part of a transfer of Care hub working alongside Advanced Nurse Practitioners, the wider community multi disciplinary team and social care to ensure that patient's are cared for on the appropriate community pathway to support their individual needs.

As a Community Sister/ Charge Nurse you will be responsible for supporting your team to deliver high quality patient care, utilising results from clinical audit to support you to do this. You will provide leadership to provide strong clinical leadership to the urgent nursing team and wider MDT acting as a role model at all times. You will work flexibly and in partnership with health and care colleagues to support effective system flow.

As part of your role you will provide effective triage and assessment ensuring patients receive the right level of care, at the right time and in the right place according to the needs of the individual and within available health and social care capacity

Main duties of the job

  • Provide skilled nursing advice, support and care to patients, their family/supporters and carers
  • Complete effective clinical triage for admissions avoidance and urgent response services including the virtual ward, 2 hour urgent response and step up and step down discharge to assess pathways
  • Utilise clinical knowledge within the Transfer of Care hub to assess and make decisions regarding patient care and safety in a crisis situation
  • Have a working knowledge of all discharge pathways and community services available.
  • Be involved in complex decision making and problem solving relating to clinical care and risk assessment on an ongoing basis
  • Promote good team working and an environment to enhance job satisfaction and ensure team members feel valued and empowered.
  • Be able to work and manage stressful situations that occur from both patient and staff dissatisfaction and be able to identify resolution for these situations. Ensure escalation of any issues when necessary.
  • Contribute to the learning and development needs of individual staff through the wider team by completing Personal Development Reviews and one to ones.
