We have an exciting opportunity for an exceptional
nurse leader to join The Rotherham NHS Foundation Trust as a Diabetes
Specialist Nurse & Service Lead. This is a key role at the heart of an
evolving, quality-driven diabetes service, focused on innovation,
collaboration, and outstanding patient care.
Youll lead a committed team of Diabetes
Specialist Nurses and work across community and acute care settings, helping
shape the strategic direction of diabetes services within the Rotherham health
community. This role combines advanced clinical practice, operational
management, and service developmentoffering a rewarding challenge for an
experienced nurse leader ready to make a difference.
Main duties of the job
Lead and manage the Diabetes Specialist Nursing team, ensuring delivery of high-quality, evidence-based care.Develop and implement care pathways aligned with the Super Six Model of Diabetes Care.Drive service innovation and quality improvement, ensuring performance meets agreed KPIs and national standards.Act as an autonomous practitioner with your own specialist caseload..Deliver expert education to staff, patients, and carers, and contribute to national structured education programmes such as DAFNE and DESMOND.Work with the Service support manager to support business planning, budget monitoring and contribute to policy development.

- First Level Degree in health / nursing related Studies or equivalent level of study
- Relevant prescribing qualification
- Post basic qualification in diabetes or equivalent portfolio as evidence
- Experience of working within the Diabetes specialist area
- NHS Leadership course or completed a recognised leadership development programme
- Demonstrable Senior management or equivalent experience

- Relevant teaching qualification
