Job summary

As a Matron for Community Specialist Services, you will lead and manage our teams to ensure a collaborated approach to patient care focussing on quality, experience and patient outcomes.

We are looking for a motivated and innovative leader, with a passion for quality patient care and a drive for a position of continuous improvement.

The NHS 10-year plan emphasises patient-centred care with a focus on the shift from hospital to community care. Highlighting the importance of multidisciplinary collaboration to develop and implement care plans that meet the needs of patients closer to home. The plan's focus on prevention and early intervention will require you to ensure compliance with policies and guidelines that support preventive care, health education, and early intervention programs to improve population health. You will be at the forefront of these exciting developments, leading your team to deliver innovative and high-quality care that meets the evolving needs of the community.

Main duties of the job

  • Your responsibilities will include overseeing the delivery of specialist services, ensuring the implementation of best practices, and fostering a culture of continuous improvement and patient-centred care.
  • Lead and manage community specialist services, ensuring the delivery of high-quality, patient-centred care.
  • Collaborate with multidisciplinary teams to develop and implement care plans tailored to the needs of individual patients.
  • Monitor and evaluate the performance of services, identifying areas for improvement and implementing necessary changes.
  • Ensure compliance with NHS policies, guidelines, and standards, maintaining a safe and effective care environment.
  • Provide leadership and support to staff, promoting professional development and fostering a positive working environment.
  • Engage with patients, families, and carers to ensure their voices are heard and their needs are met.

This role is for 30 hours per week
