Job summary
We are looking for a Paediatric Diabetes Specialist Nurse to join our excellent paediatric diabetes team in Rotherham. Our service looks after children and young people with diabetes (aged 0-19 years) in acute, out-patient and community settings. Successful in consistently achieving improving outcomes, we work as a cohesive multi-disciplinary team focused on patient outcomes. The post holder will utilise their skills of clinical expertise, leadership, education and management to ensure a seamless service is delivered. The post holder will work closely with the multi-disciplinary team in the hospital and community to ensure a co-ordinated quality service. There will be close relationships with General Practice, schools and colleges in Rotherham. You will support the development and delivery of diabetes education to patients, carers and health care professionals and assist in maintaining an effective, patient focused service that is sensitive to the needs of users and providers.
Main duties of the job
To deliver a high quality nursing care to people with diabetes within the framework of the PDSN Team.To manage a caseload of patients who have specific diabetes needs.To assist with the development and delivery of diabetes education to patients, carers and health care professionals.To assist in maintain an effective, patient focussed service that is sensitive to the needs of users and providers
