Part time - 28 hours per week
The TRFT Adult Speech & Language Therapy Service is an
integrated service assessing and treating community patients in a variety of
settings: in their own homes, care homes and outpatient clinics; as inpatients
at Rotherham General Hospital; as outpatients along Head & Neck Cancer
regional pathways; and in addition providing a Voice and Upper Airway Service
working closely with ENT colleagues. We are a small, close-knit team looking
for an experienced Clinical Specialist Speech & Language Therapist to lead
the care homes provision.
The successful candidate will have and be able to demonstrate:
* Highly developed skills in the assessment, management and treatment of
communication and swallowing disorders particularly in people with acquired and
progressive neurological conditions including dementias and/or respiratory
conditions and other long term conditions.
*Dysphagia Competencies to Specialist Dysphagia Practitioner level or above are
essential.
*Experience with AAC and Assistive Technology
*Highly skilled communication and team working skills
*Due to the nature of the role it is essential that applicants are able to
travel independently between TRFT sites and for patient visits in the Rotherham
area and have access to a suitable vehicle for business purposes. If necessary,
adjustments can be considered in accordance with the Equality Act.
The post-holders working pattern will ideally include a Monday to support duty
leadership requirements.
Main duties of the job
The successful applicant will triage and prioritise care
home referrals and arrange follow up taking into account urgency, clinical
acuity and location. The post holder will hold their own caseload assessing and
managing residents who are experiencing swallowing and communication problems
in the context of highly complex needs related to a range of acquired and/ or
progressive neurological or long term conditions.
They will link in with the
MDT around care homes and the wider community to ensure the best outcomes for
residents. As a key contact for care homes and the MDT around them, they will
be a source of knowledge and clinical expertise and the role will include
devising and providing education and training programmes in response to local
need.
They will oversee, review, and ensure application of, relevant local guidance
eg eating & drinking with acknowledged risk, end of life care, in keeping
with national frameworks and other regional services.
They will work with the other service leads to oversee the supervision and
development of relevant competencies for community based speech and language
therapists.
They will lead and/or support other team members and the wider care
homes MDT in the development of existing and new treatments and evaluate
outcomes.
The post-holder will deputise for the Clinical Lead at times of leave. There
will be a requirement to occasionally provide cover on the wards at Rotherham
Hospital.
