Job summary

The Community Staff Nurse will work Out of Hours as part of the Urgent Community Response team within the Community hub. The community staff nurse will need to be able to prioritise urgent patient care in the community setting dependent on the patients needs and clinical condition. The post holder will be required to deliver clinical assessments and care interventions to ensure that the individualised needs of the patients are maintained.

Main duties of the job

The community staff nurse working in the Out of Hours team must be able to triage patient calls utilising the appropriate triage template on system one, communicating with the patient to ensure that the appropriate care intervention is provided to patients in a timely manner. The community staff nurse will need to be able to prioritise patient care dependent on the patients needs and clinical condition. The community staff nurse will provide individualised patient care plans ensuring high standards of clinical practice are maintained through continuing personal/professional development. To support and deliver care to patients in the community setting who are on a Urgent Community Response caseload or have a urgent community response condition or illness.. The community staff nurse role is to improve clinical outcomes for patients by providing clinical interventions within the community setting during Out of Hours. This will reduce prevent inappropriate hospital admission. The community staff nurse will work closely all team members of the Community Urgent Community hub and members of the multi-disciplinary team. Work in partnership with members of the Health and Social Care Teams in the delivery of services to achieve collaborative and integrated working.
