As a Trusted Assessor you will work with patients, families, care homes and health and social care professionals to ensure the safe and timely discharge from the acute or community bed base to a care home where this is the persons usual place of residence or a new placement ensuring the needs of the individual and the care home are appropriately taken into account.
Main duties of the job
The Trusted Assessor will work with Care Home Managers and/or their nominees, the Integrated Discharge Team (IDT), Transfer of Care Hub and Health and Social care colleagues to reduce the risk of harm associated with delayed discharges.This role will identify and support the assessment needs of the individual, which may be complex, of residents either returning to their usual place of residence or to a new placement to ensure a seamless transition.The Trusted Assessor will undertake and support others in assessments which facilitate effective and timely discharges from acute hospital and community settings, ensuring people are discharged to the right setting with complete, accurate and up to date information and associated documentation.
