Job summary

A unique opportunity has arisen to join the Acute Response Team as an Advanced Clinical Practitioner. The Acute Response Team is a recently formed innovative team combining the Hospital at Night & Critical Care Outreach Teams. The ART covers a 24/7 service responding to referrals for unwell and deteriorating patients across all adult wards and departments at Rotherham General Hospital, monitoring of ICU step downs and provides trust-wide education on NEWS2 & management of deteriorating patients whilst also providing some support to the out of hours medical teams.We are seeking to recruit an experienced enthusiastic, innovative, and driven qualified clinical practitioners to the Acute Response Team from nursing or AHP background.We will support you to work within the 4 pillars of Advanced Practice (Clinical Practice, Education and Learning, Leadership and Research) with dedicated non-clinical time and an educational supervisor. Free access to the Practitioners ePortfolio will be provided to support & evidence the ACPs ongoing development.An experienced team of ACPs, Specialist Nurses and HCSWs, the medical and critical care clinical leads and on call teams will support you with your development.We will consider applications from current Trainee ACPs towards the end of training who can evidence a portfolio of learning within the 4 pillars of advanced practice.We encourage an informal discussion with all applicants. Main duties of the job

The post-holder will provide clinical expertise, leadership, collaboration and education to ensure that patients gain effective, appropriate and timely care.The ACP (post holder) will engage directly with all members of the multi-disciplinary team in the prevention and management of deteriorating patients working closely with the on call medical and critical care teams.The post holder will undertake advanced clinical assessment, initiate investigations and interpret their results so clinical diagnosis can be determined and appropriate treatment/interventions initiated.The post holder will use advanced knowledge and skills to provide autonomous practice to patients including independent clinical assessment, diagnosis, and treatment of patients at risk of deterioration, preventing admission to critical care and where required supporting that admission.
