Job summary

The Division of Medicine is looking for a passionate, hardworking and friendly Diabetes Nurse Specialist to join the Diabetes Clinical Service Unit as the In Patient Diabetes Nurse Specialist

It is a really exciting time to be joining the trust as the Diabetes in Patient Nurse Specialist to work closely with the Diabetes Clinical Lead, Matron and Service Management team to be involved in developing the Diabetes in Patient team for the trust.

The Diabetes Specialist Nurse will provide specialist advice and management to patients, their families and the multi professional team in line with national best practice and trust policies and procedures and take an active role in the development of colleagues knowledge, skills and education to ensure the care we provide for patients is safe, effective and responsive to their needs.

Professional development and clinical supervision will be provided by the trusts Lead Diabetes Nurse and there will be close links with the community Diabetes Team.

Main duties of the job

  • The role of the Diabetes Nurse Specialist is to provide an effective and high quality patient-focused service sensitive to the needs of our patients and providers.
  • To empower patients with the skills and knowledge necessary to achieve an appropriate level of independence to enable self-management of their condition.
  • To provide expert advice and specialist education to nursing and other health professionals involved in the care of people with diabetes in an inpatient setting
  • To assist in the design and delivery of education programmes for patients and carers
  • To professionally work with the Community Diabetes team to look at ways to enhance the pathway of care for our patients
