We are currently looking for enthusiastic, highly motivated Registered Stroke Specialist Nurse to work within our Stroke Unit at Rotherham Foundation Trust.

The Stroke unit have recently moved to our new purpose built Acute (ASU) and Rehabilitation Unit, which consists of 19 Stroke patients, 5 Neuro-rehab beds and 3 medical outliers.

There is an exciting opportunity to join our stroke Nurse Team working alongside the MDT to enhance the patients experience throughout the Stroke Pathway.

You will be part of a multidisciplinary team providing a diverse range of supportive nursing interventions. You will care for patients from the acute stage of their treatment to the rehabilitation stage through to community care.

We aim to provide an individualised and supportive environment for patients in which to deliver this specialised care for patients following a stroke.

Main duties of the job

The aim of ASU (acute stroke unit) is to provide holistic care for acute stroke patients, utilising a co-ordinated approach in ensuring a safe transfer from our regional Stroke centres back to ASU. You will participate in a daily conference call highlighting Stroke patients whom need to be repatriated and working closely with the Clinical Site team to ensure that Stroke beds are available.

You will visit Stroke patients in UECC and if outlying on other wards to provide that familiar face and point of contact.

Excellent communication is vital as we work with a mixture of nursing teams, medics and therapists providing input at Family based meetings to ensure all are working towards patients specific individual needs

You will asses and provide specialist stroke care to acute stroke patients and for patients requiring rehabilitation, working interdisciplinary with other professionals to achieve the best outcomes for the patients.

Italso involves leadership to ensure the stroke service runs efficiently as well as providing the development of high quality stroke care. The Stroke Nurse team are responsible in education to all Trust staff re raising awareness of Stroke symptoms and management

Work alongside nursing colleagues on the ward to achieve high standard stroke specific care and involvement in new Stroke Initiates.

Workclosely with the Consultants in the referral process of TIA clinics, booking patients in for specific scans and providing face to face consultations.
