Job summary

An exciting opportunity has arisen for a dynamic nurse with a passion for End of Life Care, as Lead Nurse in the Safe and Supportive Care Team. Full Time/Part time at Band 8a

The role is leading a highly motivated team and continuing the development of the Palliative care, Oncology and Cancer of unknown primary pathways across the acute wards and community.

Leading the development of Palliative and End of Life care across the Trust, enabling doctors, AHPs and nurses as well as non-professionally registered health care staff to develop role-appropriate critical skills and decision making in relation to end of life care

The successful candidate will have excellent communication skills to communicate effectively with patients and loved ones when receiving diagnosis and a commitment to providing high quality person centred Care

Maintaining links with professional networks and stakeholders as appropriate and to escalate concerns from operational group meetings to the strategic groups to ensure outcomes are achieved

Working clinically you will be a role model to all disciplines promoting high standards of person centred care to patients requiring End of Life care

You will be supported by Palliative Care Consultants and receive regular clinical supervision and support from a psychologist

Main duties of the job

  • Working with senior nurses in Oncology and Cancer of unknown primary service to provide a seamless service for patients
  • Working with colleagues across the acute and community service to implement the End of Life Strategy and Annual plan
  • Work collaboratively with senior clinicians to provide high quality patient centred care
  • Work clinically utilise prescribing skills to assist in symptom control and maintain comfort of patients
  • Lead on the update of existing policies and guidelines within the services to ensure staff are up to date and working according to best practice
  • Act as an advocate to patient and their families to ensure end of life care meets the needs of the patient and in line with local and national standards
  • Have an in-depth knowledge in end of life care to advise senior managers on clinical issues
  • Maintain professional links with local services to share information to provide best practice
