Emergency Medicine Clinical Fellow CESR Pathway

Job summary

The Emergency Department at Nottingham University Hospitals' NHS Trust is excited to be recruiting to our CESR (Certificate of Eligibility of Specialist Registration ) Fellowship Training Programme, offering an opportunity for a motivated doctor to join our dynamic and friendly Emergency Medicine team. Our CESR workforce currently has 5 fellows in the programme and we are looking to increase the team.

Depending on your level, upon commencement, you may begin working on our Tier 3 rota and in your first year, you will aim to experience both adult and paediatric ED. After 12 months and a review of your achievements with the CESR Lead (which will include assessing your capacity to work as a senior decision maker) we can look to progress you to the Tier 4 rota and start to plan your placements in other specialities.

If you have achieved your IAC (anaesthesia rotation) elsewhere in the NHS and have been working in a Senior Clinical Fellow capacity then we can consider your commencement on the Tier 4 rota (which is our registrar rota).Please note this in your application

Initially, we would offer a 1 year contract and after this time and a review we will look to offer a further 2 year contract extension and entry onto the CESR pathway.

Main duties of the job

At NUH, we would employ you as a Junior or Senior Clinical Fellow for the first year in the role. This enables you to have time to settle in and acclimatise to the busy department. Towards the end of this period and after review and assessments, if mutually agreed you will be able to move into the CESR programme.

The CESR programme will be expected to require around 4 years for its completion. During this time, you will work through competencies to gain the necessary skills and knowledge and develop a portfolio of evidence to meet the requirement of the Emergency Medicine CCT curriculum. We will allocate rotations through Acute Medicine, Intensive Care, Anaesthesia and Paediatric Emergency Medicine to enable you to acquire specialist knowledge and skills in these areas. In the Emergency Department you will work under the clinical direction of EM Consultants who are present in the department 24/7.

This service provided by Emergency Medicine is based at the QMC campus of NUH. You will join a twenty-four hour a day, seven days a week roster, managing adult and paediatric emergency attendances.

The Department has 24 hour Consultant cover, and the recent workforce investment has supported an increase across the entirety of our clinical establishment.

The CESR programme at NUH has run for 6 years and is extremely successful. We have already seen three of our NUH ED CESR graduates progress from trainee to Consultant here at the trust and two have become substantively employed at NUH.

Responsibilities and duties of the post include:

Working in our ED encompasses many management skills such as managing the shop floor, keeping to the four hour target and constant reprioritisation of tasks.

The assessment and provision of treatment to patients attending ED, liaising with appropriate specialties for ongoing management.

Acting as resuscitation team leader for seriously ill or injured patients, including children, presenting to ED.

Liaising with the shop floor consultant in charge for clinical and administrative advice as appropriate.

Working with the nurse in charge and the wider multi-professional team to ensure effective running of the department, supporting the delivery of safe, timely, quality care, in line with the National Emergency Care Standards.

Contributing to the provision of an effective emergency medicine service in collaboration with colleagues in the multi-professional team.

The supervision and training of junior doctors, Advanced Practitioners, Physicians Associates, Urgent Care Practitioners and Emergency Nurse Practitioners.
