Job summary
We are looking to recruit a dynamic and motivated individual to our Renal Anaemia Service who has a passion for optimising care for adults with Chronic Kidney Disease (CKD)
You will have at least 3 years post-registration experience of nursing, preferably within a renal clinical setting, and hold a V300 independent prescribing qualification. Consideration may be given to those working towards it
The successful applicant will independently review, assess and amend patient medications in the management of anaemia of CKD and will work in collaboration with members of the multi-professional team and external agencies in co-ordinating a smooth and efficient pathway of care for our renal patients. You will be an expert resource to patients, carers and clinical teams in relation to anaemia in CKD within our services
You will working within a small, friendly and supportive team of specialist nurses. Your role will include working autonomously, managing a caseload of patients across our renal services. Primarily based at Nottingham University Hospitals City Campus you may also be required to travel to Kings Mill Campus. Your shift pattern will be determined by negotiation and can be flexible at times

Main duties of the job

To work within a specialist nursing team, maintaining and developing a quality service for patients receiving renal care
You will manage, develop and ensure delivery of high-quality care. You will act as a clinical expert within the field of anaemia in Chronic Kidney Disease (CKD). You will support the Band 7 Renal Anaemia Nurse Specialists in providing an anaemia management service for adult patients with CKD and Heart Failure. You will also co-ordinate with SDEC services and administer IV Iron/Erythropoietin Stimulating Agents to our renal patients where needed
Take responsibility for the care of a caseload of patients in outpatient settings
Administer IV Iron and S/C Erythropoietin Stimulating Agents, teach and support patients and carers with anaemia treatment. Review, analyse and interpret lab results in order to analyse treatment requirements and evaluate treatment efficacy
Independently make dose changes to drugs used in the management of anaemia of CKD for patients who have already been initiated on treatment for their anaemia of CKD
To provide clinical expertise and skills in the assessment, implementation and evaluation of anaemia treatment plans. Collaborating with other members of the multi-professional team both within and outside the specialty, where appropriate, to ensure best outcomes
