Job summary
Working with a vision of "delivering excellence in all everything we do", the post holder will be based within the Transition of Care Units. The post holder will be expected to be able to work flexibly across QMC and City Hospital campuses.The post holder will be required to:Manage all aspects of patient flow and capacity across the Transfer of Care Units (TOCUs) at QMC and City campus as bed management for bothWork as flow and capacity based within the discharge lounge at QMC mainly ensuring that there is constant flow into and out of the area, ensuring that all patients are discharged safely and in a timely manner transport and care call times are met.Ensure that the safety of the patients is paramount by assisting in the seamless movement of patients throughout the Trust and onwards.Be effective in managing challenging flow and capacity issues which may need further escalation to Specialty Management Team or the Directorate Leadership Team

To drive through the delivery of NUH flow and capacity targets to continually improve performance within the service.To provide operational expertise and support to all members of the serviceTo work closely with the ward managers /leads of the wards/areas within Directorate around flow and capacity.On a daily basis develop and review plans to achieve patient access targets at service level.To identify the impact of changes to services or increases in services on resources and negotiate changes in resource where appropriate.To manage and monitor changes to the administrative systems of the service (e.g. , NerveCentre, Careflow )To contribute to service improvement at local and Trust level.To provide and receive complex and highly sensitive information where objections need to be overcome to deliver the best outcome for patientsTo work alongside site ops teams and divisional on call teams and respond proactively to ever changing demands in line with current system statusTo drive timely flow from base wards and other areas to TOCU to support flow and capacity within ED.Work with ED around the placement of any Domestic Violence (DV) patients.Act as resource for staff around the management of DV patientsTo ensure cross-campus working to deliver flow and capacity over both sites.
