Job summary
This is an exciting opportunity for a Discharge Coordinator to join a diverse and motivated team that are dedicated to the development of skills and the delivery of high quality safe care on C31.
Ward C31 is a busy Emergency Surgery admissions ward dealing with a wide range of general surgical conditions.
We are looking for an individual who is self motivated, content with working in a busy admissions environment and who has good time management skills. You must have excellent written and oral communication skills with the ability to work autonomously and as part of a team. It is essential that you act professionally at all times, have excellent interpersonal skills and the ability to prioritise your own work load.
Main duties of the job
This person is expected to work in the ward environment, alongside the MDT to ensure that effective discharge planning and communication are managed in order to provide a smooth, timely discharge from hospital.
Job Purpose:
- To work with clerical and clinical staff to maximise the efficient and effective utilisatyion of the Trusts beds.
- To have responsibility for co-ordination of the ward's discharge process
- To ensure patients, their carers and members of the MDT are fully involved at all stages of the patients admission / discharge process.
- To utilise IT/information within the trust to facilitate better discharge on the ward and enable the ward to alter processes to improve discharge.
