Job summary
We are looking to recruit a motivated energetic and experienced renal nurse into the Renal Home Therapies Team supporting patients at home whilst carrying out their own dialysis treatments. The post holder will care for a cohort of patients, working mainly 8am-4pm Monday to Friday, but with flexibility to work in the evenings when required. They will also be on-call at weekends (one in six weekends) to support the Assisted PD Team.
All applicants must hold a UK driving licence and have access to a reliable car for work.
Main duties of the job
- To teach patients how to carry out their own Peritoneal Dialysis treatment at home and to provide specialist nursing care and support to patients and their carers at home, maintaining and developing a quality service.
- To support patients and their carers carrying out Haemodialysis at home.
- To visit patients who are approaching the need for dialysis and educate them about their choices around dialysis treatments
- To take overall responsibility for the care of a caseload of patients requiring specialist nursing care within the hospital and home setting.
- To provide specialist training, to both staff and patients and their carers, as required.
