Job summary

An exciting opportunity has arisen for an experienced oncology nurse to join the Colorectal Oncology Clinical Nurse Specialist Team on a permanent basis. The Colorectal Oncology CNS team provide keyworker support, advice and onward referral if necessary, for all patients undergoing oncological treatment for their Colorectal Cancer at Nottingham University Hospitals NHS Trust and Sherwood Forest Hospitals Foundation Trust.

The successful candidate will work with the Colorectal Oncology CNS team to co-ordinate the care of patients with Colorectal cancer in partnership with other nursing staff and psychosocial services to assess and establish the needs of the patient.

The Colorectal Oncology CNS team have a strong vision for service development in the future, and are seeking an enthusiastic and highly motivated individual with excellent communication skills, and a strong team ethos, to join them on this journey.

Main duties of the job

To act as the key worker for patients with Colorectal cancer requiring oncological treatment, ensuring specialist assessment of need and referral to appropriate support services.

To work closely with other multi-disciplinary teams across the Trust to provide specialist nursing advice.

To co-ordinate the care of patients with Colorectal cancer in partnership with other nursing staff and psychosocial services to assess and establish the needs of the patient in various clinic settings. This could include assessment of symptoms, psychosocial care, care outcomes, co-ordination of patient pathway.

To plan patient care in partnership with the Multi-disciplinary team, for example surgeons, oncologists, psychologists, social workers and palliative care teams

To communicate treatment information to patients and carers, and support the Multi-disciplinary team in the management of complex cases, by acting as an independent practitioner to monitor the patient's condition.

To act as the specialist resource and specialist advisor for all professionals involved in the care of patients coming through the colorectal oncology service.

To signpost and establish support networks for patients and practitioners including social / support networks.

To influence and develop policy making and clinical guidelines to support the service.

To implement, maintain and update nurse-led clinics in accordance with local and national developments.
