Job summary

There is an exciting new opportunity within the Neonatal Home Care Team. We are seeking a dynamic, enthusiastic, motivated and experienced Band 5 Neonatal Nurse to be part of our Home Care team helping to provide a comprehensive service to maintain our 7 day working and contribute to the creation of a network-wide outreach service.

Band 5 Home Care Neonatal Nurses based at Nottingham University Hospitals NHS Trust (NUH) are required to promote safe early discharge of babies from the Neonatal units. Families will be supported to provide ongoing care needs in the home environment, for example the provision of nasogastric tube feeding at home, and delivering home oxygen.

The NUH Neonatal Service provides care for some of the most critically unwell infants in the East Midlands Neonatal Operating Delivery Network (EMNODN) as a Level 3 centre, providing neonatal surgery within and beyond network boundaries. This post is a permanent part time Band 5 vacancy available at NUH to support early repatriation, create education packages for parents and staff within EMNODN as a neonatal outreach service able to provide surgical support.

Candidates must have experience of caring for neonates in a hospital or community environment, and must be, or working towards QIS trained in speciality. This post offers a fantastic opportunity to utilise and develop your knowledge and skills in caring for babies in the home setting.

This post will support the existing outreach team in planning the care and transition from the neonatal unit through a structured discharge process, promoting safe early discharge from hospital to home. You will liaise with other members of the multidisciplinary team and support parents in providing high quality care in the home environment.

The service is run over a 7 day period, successful candidates will be expected to be on the weekend rota once a period of induction has been completed.

Candidates will be expected to undertake a regular clinical shift on either unit, equivalent to 10% of their weekly hours.

As part of the Neonatal Service at NUH, the post holder will assure that all babies (who fulfil the stated criteria in the Neonatal Service Home Visiting Guidelines) receive access to the home visiting programme after discharge from the neonatal service.

The post holder will participate in the discussion around discharge planning with the parents/carers of the baby, the medical team, low dependency team and wider multidisciplinary team prior to discharge of the baby and then facilitate the documented home care plan for this baby at home until such time that the baby is deemed fit to transfer care to the relevant health visiting team and wider community.

The post holder will act to ensure that the standards and quality of care given to babies, together with the environment in which care is delivered, are maintained at a high standard. The post holder will be expected to facilitate and co-ordinate home care and family centred care.
