Stroke Discharge Coordinator

Job summary

An excellent opportunity has arisen for a discharge coordinator to join our Stroke team at QMC campus. The post holder will be based on the new Queens Stroke Unit, working throughout the stroke pathway from admission to discharge. Your role will be to coordinate inpatient flow across the three wards in stroke services, including Daybrook ward at City Campus. You will work in collaboration with the Medicine bed managers and the SMT (specialty management team - head of service, matron and specialty general manager) as well the wards staff and MDT team.

The aim of the role is to ensure timely transfers from the emergency department, discharges and transfers and that all patients are tracked through their stay and escalate any issues that delays along a patient's journey and prevents discharge, working as required to minimise these.

A proactive approach is required to pre-empt delays where possible by using their knowledge and understanding of stroke services and hospital systems such as Medway and Nerve Centre in order to monitor the patient flow and identify issues. The post holder is expected to use hospital systems in the use of discharging and transferring patients. The post holder will inform the Nurse in Charge and Consultant in Charge of any internal escalations that hinder patient flow through the departments.

The Stroke Discharge Coordinator role is seen by all clinical and nursing staff as an integral role to the patient journey and support to staff in Stroke.
