Job summary
We are currently looking for an enthusiastic and dynamic pharmacist to deliver a specialist clinical pharmacy service to the Trusts newly opened Transfer of Care Unit (TOCU) and cardiology/health care of older people wards at Nottingham University Hospitals NHS Trust. This role provides a new exciting opportunity to shape the pharmacy service delivery to TOCU and to work alongside the specialist cardiology pharmacists to develop your clinical cardiology knowledge and expertise.
The Transfer of Care Unit works closely with both the cardiology and HCOP wards and your role will be vital to provide a high level of pharmaceutical service to inpatients, improve the quality of discharge communications and facilitate the smooth transfer of medication related issues across the interface at admission, discharge and transfer. The role also provides an excellent opportunity to work across the wider Nottingham Integrated Board and the potential to participate in optimising medication for patients on 'Virtual Wards' in the future.
You will form part of our experienced and expanding Cardiology pharmacy Team and will have mentoring support. You will also be joining the trust at an exciting time due to the recent implementation of electronic prescribing and Medication administration(EPMA).
Hospital experience is desirable but not essential and we welcome applications from pharmacists with a interest in settings and cardiology. Inhouse training and support will be provided.

To provide a clinical pharmacy service to the new TOCU and cardiology/HCOP wards, offering highly specialist pharmaceutical advice to medical staff, nurses and patients on all aspects of safe and effective use of medicines.
To improve the quality of discharge communications between secondary and primary care (community pharmacies and general practice) and to care homes.
To work alongside the cardiology & HCOP multidisciplinary team delivering excellent patient care through medicines optimisation. When qualified the post holder will also utilise their prescribing skills to do this.
To assist the specialist clinical pharmacists with specialty level support roles such as governance, teaching and guidelines.
To be actively involved in quality improvement projects and business cases.To support, mentor & line manage junior pharmacists and medicines management technicians.
To work alongside the Cardiology multidisciplinary team delivering excellent patient care through medicines optimisation. When qualified the post holder will also utilise their prescribing skills to do this.

To ensure safe, rational and effective prescribing by undertaking medication reconciliation, assessing suitability of prescribed medication, enabling use of patients own medicines and patient self-administration, counselling (to optimise medicines use), and ensuring continuity of patient care on leaving hospital.

To support with proactive assessment of patient adherence and ability to manage medicines prior to discharge; working with patients as partners ensuring their autonomy where possible.
To improve the quality of discharge communications between secondary and primary care (community pharmacies and general practice) and to care homes and to actively identify and/or refer patients to benefit targeted NHS Discharge Medicines Service to a community pharmacy.
To support with Quality Improvement Project work focused to improve patient flow and timely discharges.
To partake in clinical pharmacy duties ensuring core clinical pharmacy activities are undertaken in the relevant clinical specialties. This includes a prescribing role when qualified.
To make recommendations about medicines to improve patient outcomes and to encourage adherence to hospital formulary and national guidelines.
To line manage and support more junior pharmacists and pharmacy technicians in the medical team specialties.
To facilitate or conduct clinical audits which are part of the pharmacy audit plan that are suitable for presentation at a local level.
To take full part in the 7 day shifted rota and bank holidays duties.
