Senior Clinical Pharmacist - Healthcare of the Older Person

Job summary

We are looking for an enthusiastic clinical pharmacist to join our friendly and expanding pharmacy team caring for patients in our Healthcare of the Older Person (HCOP) wards.

We have a team of Specialist Frailty Pharmacists who will offer supervision and training in how to provide excellent care for our older patients. This will allow you to build up your knowledge and skills to give you a wealth of experience for your CV.

You will provide complex, pharmaceutical care for patients, optimising their medicines and ensuring the safe & effective transfer of care, and have mentoring support of Specialist Frailty Pharmacists, and operational support from junior pharmacists and medicines management technicians. You will work with the wider multidisciplinary team including specialist nurses.

  • To work alongside the HCOP MDT delivering excellent patient care through medicines optimisation
  • When qualified/met Trust validations the post holder will also utilise their prescribing skills
  • To assist the Frailty Specialist Clinical Pharmacists with speciality level support roles such as governance, teaching, updating/creating guidelines
  • To be actively involved in quality improvement projects and business cases
  • To support, mentor and line manage junior pharmacists

Essential

  • Honors Degree in Pharmacy/B.Pharm/BSc(Pharmacy)/MPharm
  • Registration with General Pharmaceutical Council (GPhC)
  • Demonstrates continued professional development

Desirable

  • Member of Royal Pharmaceutical Society (RPS)
  • Non-medical prescribing
  • Post-Graduate Diploma/MSc in Clinical Pharmacy or working towards this

Experience

  • Wide previous experience (post-registration) in clinical pharmacy
