Job summary

An exciting opportunity has arisen for an enthusiastic, highly motivated, registered nurse to take up the role of the access nurse in the renal unit at City Campus of Nottingham University Hospitals

The post holder will be will be based at Nottingham University Hospitals City Campus. The role covers Bramley ward, Carrel ward, City Dialysis Unit, Centenary Wing, Ilkeston Dialysis Unit, Kings Mill Dialysis Unit and South Nottingham Dialysis Unit. The role may require travel to/from the satellite dialysis units and Ilkeston, Kings Mill and Lings Bar. The renal nursing team is cohesive and professional with a genuine interest and passion for providing a high standard of dialysis care.

Main duties of the job

The post holder must have extensive renal experience and have the desire to be an innovative leader, supporting and developing learning. The successful candidate will work as an integral part of the multidisciplinary management and clinical teams. They will demonstrate clinical expertise and leadership in the delivery of care to patients requiring vascular access for renal replacement therapy .

You will have responsibility for ensuring that patients are fully involved in their care, provide evidence that patient feedback is used to maintain standards or implement improvements that have an effect on the care that patients receive.

Be an effective role model in providing direct clinical care in addition to ensuring effective use of non-clinical time for clinical governance, key performance indicators.

The Nottingham Renal unit supports patients undertaking haemodialysis in an outpatient hospital setting and is the centre for renal transplant with 2 inpatient wards. Job responsibilities

The post holder will deliver a patient centred service which ensures all adults with advanced and established chronic kidney disease have timely and appropriate surgery for permanent dialysis access which is monitored and maintained to achieve maximum longevity. Special responsibilities include surveillance and on-going monitoring of existing access, liaising with nursing, medical, surgical and interventional radiological teams to maintain and increase access survival in line with national standards.

Establish and maintain evidence based protocols for a patient access pathway for referral and treatment. Teaching staff and patients about vascular access. Visiting satellite units to teach and troubleshoot about vascular access.

The post holder will be able to assess the physical, social and psychological needs of patients and to plan individual patient care in relation to elective admission accordingly.

The post holder will provide pre-operative, intra-operative and post-operative information to the appropriate persons in the nurse led clinics.

The post holder will ensure a patients journey is as seamless as possible to theatre and cancellations of procedures are minimal.

Establish and maintain good working relationships within the multidisciplinary team and to work collaboratively and co-operatively with clinical colleagues across the Trust to enhance quality of care and share best practice.

Work unsupervised and give advice to juniors regarding suitability for surgery
