Muscular Dystrophy Care Advisor - Paediatrics

Job summary

This post is for a fixed term period until end of May 2025.

The successful person will be responsibility for the coordination of specialist, complex support of children/young people with a diagnosis of Muscular Dystrophy.

They will be a single point of contact for the cyp and their family and also other health professionals involved, assisting with advice and navigation of the services available, coordinating access to ongoing assessments and services, and to ensure the patients care objectives are met in order to support patient choice.

This is an exciting opportunity to join the specialist team at Nottingham Children's Hospital and is suitable for a nurse or allied health professional therapist to work with this cohort of patients.

Main duties of the job

Be a single point of access regarding information and advice for those cyp affected by neuromuscular conditions, their families/carers and the professionals working with them.

Work alongside the clinical nurse specialists/AHP's to provide effective co-ordination of hospital and community care for children and young people with MD.
