An exciting
opportunity has arisen to develop and deliver prehabilitation and
rehabilitation pathways across Barnsley and help thousands of patients across
South Yorkshire.
Active Together
is a prehabilitation and rehabilitation service supporting people following a
cancer diagnosis with physical activity, nutrition, and wellbeing guidance. The
service has been designed by Sheffield Hallam University who work in close
collaboration with NHS teams to support the delivery and evaluation of the
service.
The Active
Together service launched in Sheffield in 2022 and is currently expanding to
new sites across South Yorkshire in Barnsley, Rotherham and Doncaster. Active
Together South Yorkshire will provide equitable, multi-modal and personalised
prehabilitation and rehabilitation support that meets the needs of patients
across the region and generates evidence for a commissioned service.
Barnsley
Hospital Foundation Trust and Barnsley Premier Leisure are working in
partnership to deliver this programme to the population of Barnsley.
Active Together
is funded by Yorkshire Cancer Research, Yorkshires independent cancer charity,
working so more people can live longer, healthier lives, free from cancer.
Main duties of the job
To be the managerial and
clinical lead, incorporating Trust wide accountability and responsibility for
the leadership, provision and development of a patient focused, clinically and
cost-effective specialist Therapy service for Active Together Programme.
The service will be provided
in line with Trust strategic objectives, national and local performance
targets, and governance and quality standards.
To lead collaborative working
across Therapy services maximising the utilisation of the skills of the Active
Together team which comprises registered and
unregistered therapy staff.
To promote strong
relationships between health and social care staff to ensure that
multi-disciplinary teamwork is facilitated.
To act as the representative for Active Together services within the
trust. Liaise with other therapy service managers, the Professional Lead for
Therapy and other CBU3 staff and all members the Therapy CSU team, to lead service development, improvement
and redesign, alongside maintaining a small manageable clinical caseload
