We are looking to recruit an enthusiastic, well motivated team member to work within the Discharge and Patient Flow team to facilitate safe and effective discharge, gather and input data, bed states etc. Must have excellent communication skills, willing to learn new skills and have some understanding of safe discharge and flow.
Main duties of the job
The Role:
This interesting and busy role has two elements to it:
Patient Flow -Supports the Site Matrons with facilitating safe and effective bed movements and discharges within the Trust, supporting the daily bed meetings, data gathering and inputting, submitting external SITREPs to NHS England, completion/updating of e-forms etc and producing system access for NHSP staff.
Discharge -Supports the Case Managers and involves working alongside team members to ensure safe discharges are followed by the ward, co-ordinating incoming/outgoing telephone calls and email communications between wards/departments, recording data accurately and liaising with external partners.
It is important that candidates have excellent communication skills, a willingness to learn new skills and have some understanding of patient flow and safe discharge. Candidates must have the ability to follow instructions and work on their own initiative when required.
