Job summary

We are looking for a highly skilled, motivated and enthusiastic member of staff to work as part of the Cancer Services team to deliver the personalised care agenda on a Band 4 permanent basis.
We are looking for an individual with experience and knowledge around cancer needs and who can demonstrate career experience at Band 3 / 4; from a background which reflects high levels of practice, and the ability to work with others and communicate effectively at all levels.
Whilst being an essential team player, you will be required to be self-motivated and able to prioritise your own needs and time and be able to co-ordinate care with the appropriate teams.

Main duties of the job
The postholder will work patient facing, and will provide personalised care needs for the patients who are supported by the Lung and Urology clinical nurse specialist teams. The Macmillan Cancer Support Worker will liaise with additional services to manage the patients needs, signposting to support any additional factors around holistic care needs such as; wellbeing, finances, physical, social factors and emotional care.
The Macmillan Cancer Support Worker will conduct electronic holistic needs assessments (eHNA) and navigation for these patients, signposting to our community co-workers and other agencies to give the best support available to our cancer patients.
A brief outline of the duties include:
Always maintain a high standard of work
Complete individual holistic needs assessments for patients who are diagnosed with cancer
Important to make sure that patients receive individualised personal care that suits there needs, asking the question what matters to them.
Be able to have good effective organisation skills and a knowledge of what outside agencies can do to support
Be patient and compassionate to our patients but also to their families and carers
Be able to solve problems and go the extra mile for people diagnosed with cancer
Be helpful and a team player when working within the cancer clinical teams
Have a knowledge of Treatment Summaries and Risk Stratified Pathways
