30 - 37.5 hours per week
As an
Advanced Clinical Practitioner within Trauma, Orthopaedics, and Frailty
services, you will provide advanced clinical assessment, diagnosis, and
management for patients presenting with musculoskeletal injuries and
frailty-related conditions. You will work autonomously and collaboratively
within the multidisciplinary orthogeriatric team to deliver holistic,
patient-centered care.
We are seeking a candidate who:
Holds a Masters degree or
equivalent in Advanced Clinical Practice
Has proven experience in trauma
and orthopaedics, with demonstrable knowledge or experience working within
frailty and orthogeriatric services
Demonstrates advanced clinical
reasoning with comprehensive understanding of musculoskeletal conditions and
frailty syndromes
Is skilled in advanced diagnostic
and therapeutic procedures relevant to the specialty
Possesses excellent
communication, leadership, and multidisciplinary teamwork skills
Holds current professional
registration (e.g., NMC, HCPC) and adheres to high professional standards
Is committed to continuous
professional development and evidence-based practice
Supporting and mentoring junior
staff and students
Main duties of the job
Key
responsibilities include:
Working closely with the
orthogeriatric team to assess, manage, and coordinate care for frail patients
with musculoskeletal trauma and orthopaedic conditions
Conducting comprehensive clinical
assessments and developing integrated management plans for trauma, orthopaedic,
and frail patients
Identifying frailty syndromes and
implementing tailored care pathways to improve functional outcomes and reduce
complications
Ordering and interpreting
relevant diagnostic investigations, including imaging and laboratory tests
Performing advanced clinical
procedures relevant to trauma and orthopaedics
Collaborating effectively with
geriatricians, surgeons, physiotherapists, occupational therapists, nursing
staff, and other healthcare professionals
Leading patient education,
rehabilitation, and discharge planning with a focus on frailty prevention and
optimisation of recovery
Participating in clinical
governance, audit, research, and service development within the orthogeriatric
and trauma/orthopaedic services
