Job summary
This is a new post within
Orthogeriatrics, the service is expanding into Surgical
Liaison giving opportunity for the successful candidate to shape and
develop this service. The aim is to meet the criteria stated in The NELA. The post will aid the further development of services to older
people, complementing our Acute Frailty and Re-enablement Services.
We are looking for an individual to bring their skills, knowledge and drive to our existing teams.
The post is a 10 PA post with opportunity to participate in specialist interests or
non-clinical activities such as QI..
The department has an established reputation and benchmarks
well in National audit programmes; a dedicated Acute Stroke Unit; a dedicated landing to provide acute
inpatient beds for Elderly Medicine. There is a recently established acute
Frailty unit where we provide early MDT, 4 Frailty ANPs, 1 SHO and 1 SAS working with urgent therapy team to provide same day therapy input
and same day access to community services to support same day discharge.
The wider Geriatric Department
has physicians with special interests.
You will receive mentoring
and support for this role. Undergraduate and postgraduate teaching will be
expected, the College Tutor is a member of the CSU and the Trust has a good
record in getting candidates through PACES.
Applicants must be on the GMC Specialist Register or
within 6 months of attaining Specialist Registration at the time of
interview.
Main duties of the job

areas namely ward 33 and 31/32 respectively.
This
post does include a GIM on call commitment, approximately 1:4 weeks (midweek)
and 1:8 weeks (weekends) with a mandatory rest day for weekend working (on
the Monday immediately post on call weekend). Seven days a week working is currently being
reviewed by the Trust and if agreed, we expect the successful candidate to
take a part if required, where Consultant numbers and job plans allow. However exceptions will be considered based
on candidate circumstances.

Job description
Job responsibilities
An
exciting opportunity has arisen to join and expand further our dynamic and well
respected Orthogeriatric team, and, to participate in service transformation through
the introduction of a new post Consultant
Orthogeriatrician/Surgical Liaison. Orthogeriatrics
provide an outstanding geriatric service mainly within Wards 33 and 34 Trauma
& Orthopaedics.
The current Orthogeriatric service at
BHNFT consists of a Consultant led daily ward-round reviewing all new patients
and overseeing their care jointly with the orthopaedic teams through the peri-operative
period and into rehabilitation as necessary. The Consultant Orthogeriatrician
is supported by a team of Advanced Nurse Practitioners (ANPs).
Scope
This
new post will support the implementation of a geriatric surgical liaison model
which will include:
- Peri-operative medical-surgical ward rounds:
- Daily ward rounds will be conducted on selected
patients by the Consultant Orthogeriatrician/Surgical Liaison accompanied
by members of the surgical team. Patients requiring review will be
identified by medical, surgical, nursing and therapy staff on an
open-access non-age-based policy. Indicators for medical review may
include advanced age, severity of acute illness, post-operative
complications, failure to recover rapidly or discharge delays. The aim is to meet the criteria stated
in The National
Emergency Laparotomy Audit (NELA).
- Post-operative discharge planning meetings:
- Multidisciplinary team working, planning difficult
discharges and co-ordinating team. Attendees will include a Consultant Geriatrician,
members of the surgical team (ranging from FY1 to SpR), ward sister,
occupational therapist, physiotherapist and discharge coordinator. The
meeting will identify barriers to discharge and will delegate
responsibility to team members to resolve any issues. Projected discharge
dates for all surgical patients will be established alongside enhanced
recovery principles.
- Inpatient subacute care and rehabilitation:
- Selected patients whose recovery is complicated by
medical complications or increased rehabilitation needs will have access
to geriatrician-led surgical rehabilitation. This will facilitate
transfer of subacute patients out of the acute surgical bed base.
Although parent surgical teams retain
responsibility for surgical aspects of care and emergency out-of-hours cover, support
will be provided by the Consultant Orthogeriatrician/Surgical Liaison with
appropriate discharge planning. Surgical teams will review where needed on an
as-required basis.

department also has an outpatient area (Assessment and Rehabilitation Unit)
with a long tradition of providing multidisciplinary care for patients with falls
and bone health related issues. In
addition to this the ARU also runs tilt table clinics, syncope clinics,
continence clinic, general medicine and geriatrics. We are keen to continue to
develop our outpatient care and the successful candidate is welcomed to participate
in outpatient care redesign and delivery to suit the specialist interest of the
appointee.
The
successful candidate is also expected to play a role in service development
projects and audits in the department and trust wide with a focus on improving
care for older people at Barnsley.
There
is also the exciting opportunity for the successful candidate to become
involved in our new QI Hub (Quality Improvement) with a focus on improving quality
and services within the department and trust wide. Opportunity also exists for candidates to
pursue other specialist interests relevant to the role and/or wider
organisation.
There
is also access to a falls prevention team, dementia nurse specialist, delirium
lead and there will be a scope for cooperation with the successful candidate to
contribute to further improving those services. It also worth mentioning that
we are four years into the National Dementia Strategy and the successful
candidate can build on the work already achieved by the strategic working
group.
Traditionally
the department is committed to carry length of stay rounds on medical and
surgical patients once weekly to support colleagues in other clinical area in
managing LOS patients with the successful candidate expected to contribute to
this work stream.
Within
the overall Orthogeriatric and Care of the Elderly departments there is an excellent
tradition in medical education and organising and delivering PACES exam. Dr A Eltrafi,
PhD is a previous college Tutor and a winner of multiple teaching awards. He
can offer you support in your teaching career.
Additionally,
as the Department has strong academic links the successful candidate will be
supported to develop their research skills and develop a research
interest. Honorary Clinical Senior
Lecturer status at the University of Sheffield will be requested for the
successful candidate. There is a strong commitment to education and training both
in the Trust as a whole, and the post holder will be expected to contribute to
this and to the educational supervision
of up to two junior doctors with CME built in to the timetable of this post.
