Job summary

We have an exciting opportunity for a passionate Physiotherapist with highly specialist skills and knowledge in the field of frailty and geriatrics to apply and join the new integrated Frailty Virtual Ward team. The team is a part of an integrated system between BHRUT & NELFT, both known for their work in the field of diversity and inclusivity. This service will be the first of its kind bringing integration between the Acute and Community providers in the London boroughs of Barking & Dagenham, Havering, Redbridge and Waltham Forest.

Main duties of the job

1.1 To provide specialist standard Physiotherapy service to patients within the Frailty Virtual Ward which is delivered predominantly within the patient's normal place of residence, facilitating and maximising patient potential within the service boundaries using a holistic approach.1.2 To give highly specialist guidance, advice and education to other members of the Multidisciplinary Team (MDT) on matters relating to your specialist area.

1.3 Provide leadership for your team (in conjunction with the Service Manager) in planning, coordinating, delivering and evaluating the Physiotherapy service provided to patients within your area.

We're looking forward to introducing an electronic patient record next year. This will mean the records of any patient visiting one of the seven hospitals run by BHRUT and Barts Health will be accessible to the clinical teams. It'll make things easier for staff and will be better for patients.
