Job summary

We are seeking Clinical Fellows to join the dynamic, motivated and innovative Geriatrics and Frailty team at BHRUT to deliver excellent and safe care for our patients. This appointment will be for a 12-month fixed term initially to work in our Geriatrics team with a unique and innovative Acute Frailty Unit that is among the first in the country to operate an independent Rapid Assessment and Focused Treatment (Rafting) model outside an ED environment.

The post holder will drive forward developments, improving the quality of care delivered to patients with a focus on maximising peoples' independence and supporting their return to their usual place of residence wherever possible. You will receive peer support from staff working across the Care of Elderly services at Barking, Havering Redbridge University and be fully supported by an integrated management team helping you to drive forward change.

The successful post-holder would support the development and clinical transformation of our busy Care of Elderly faculty through the extremely busy frailty pathway. We have opportunities for individuals who are looking to develop their skills at a higher level relating to the complex range of cases seen through a large number of acutely unwell elderly patients.

All staff will have a mentor for help and guidance. This post would provide a good platform for candidates wishing to seek approved training rotations, particularly in Specialist Medicine, Emergency Medicine and GP.

Main duties of the job

CLINICAL

  • Attend daily consultant-led post-take ward round.
  • Maintain overview of all ward patients.
  • The post holder will be expected to prioritise and time-manage effectively according to the activity of the department.
  • Actively participating and facilitating the discharges
  • Liaise with ambulatory care nurses if necessary.
  • Contribute in the electronic and verbal handover of patients.
  • Work a pattern of Standard Days, Long Days and twilight shifts across a 7 day roster
  • Participate in audits and research.

ACROSS ALL POST

  • Responsibility to work as a team to provide the best care to our patients.
  • Joint responsibility with junior team to plan discharges and write discharge summaries in a timely manner.
  • Provide teaching to nursing and/or ancillary staff and undergraduate students.
  • Perform duties in occasional emergencies and unforeseen circumstances at the request of the appropriate consultant, in consultation where practicable with their colleagues both senior and junior.
  • Actively participate in performance appraisal as per Trust guidelines.
  • Participate in audits and research as arranged by the firms.

Under the leadership of our Chair Jacqui Smith and Matthew Trainer, our Chief Executive, we provide care for the residents of three of the most diverse London boroughs. We know we have more work to do to improve waiting times for urgent and emergency care, and our performance against the four-hour emergency access standard remains challenged, in comparison to many other London trusts.

We are excited to be advertising a number of Geriatrics and Frailty Clinical Fellow posts, based in our newly formed Acute Frailty Unit at BHRUT. You will be joining a unique and innovative Acute Frailty Unit that is among the first in the country to operate an independent Rapid Assessment and Focused Treatment (Rafting) model outside an ED environment. The Units accept direct ambulance conveyances of older people and thereby bypass ED. The Units function as medical ED for the over 75s and ensure that all our patients undergo a Comprehensive Geriatric Assessment (CGA) by our MDT based on our units.

This post is part of an expansion of our Basic Grade tier of doctors, and is designed for candidates who are working at the level of Junior Doctor in Elderly Care, or who are working towards Middle grade and wish to advance their career in this exciting specialty. The post will provide the appointed clinician with the opportunity to gain further experience working in Geriatrics in the NHS, seeing the full range of older patients attendees.

  • The Queens Frailty Unit will have medical cover with two or more doctors present at all times. There is extended consultant cover from 08:00 21:00 daily , weekends & bank holidays
  • Seeing and assessing all older people over 75 years that enter the unit
  • Liaising with community set ups to get patients back home quickly and safel
  • The post holder will be expected to prioritise and time-manage effectively according to the activity of the department
  • Education is an important part of the role. The successful applicant will be expected to receive and deliver teaching
