Job summary
Do you want to be part of the revolution in patient Supported Self-Management that will provide a dramatically different approach to follow up care? We are transforming the way we deliver post treatment Cancer Care at BHRUT. Our vision is to support our cancer survivors to live well beyond their treatment, close to home with just the right amount of contact with us that feels right to them. We are looking for a Support Worker to join our Personalised Stratified Cancer Follow Up (PSFU) team and help us realise this vision.
The successful applicant will ramp up the delivery of PSFU across several cancer services. The post holder will work collaboratively with the PSFU Nurse, Personalised Cancer Care Team, and clinical teams to ensure that the stratified pathways are delivered. They will ensure that patients receive personalised, holistic supportive care through their pathway. The post holder will have an opportunity to support with Trust Cancer Wellbeing events, education, health promotion and will develop links with wider community services. You will be supported in your development by the ACCEND programme for cancer professionals and access to Macmillan Professional Development.
Main duties of the job
The post holder will be responsible for supporting the RMS nurse with all elements of Personalised Care provision for patients on a PSFU pathway. They will be expected to undertake duties related to the monitoring of patients registered in the Remote Monitoring System, including escalation of variances, generating letters and appointments. They will also undertake patient facing duties including aspects of Holistic Needs Assessments and Treatment Summaries in conjunction with CNS's.

We also provide outpatient services at Brentwood Community Hospital, Barking Hospital, Loxford Polyclinic, and Harold Wood Polyclinic. We have two of the busiest emergency departments in London.
As we recover from Covid-19, we're proud to be leading the way nationally in reducing the time our patients wait to get the treatment they need. The pandemic provided the drive forour teams to innovate and change.
We know we have more work to do to improve waiting times for urgent and emergency care, and our performance against the four-hour emergency access standard remains challenged, in comparison to many other London trusts.
We are particularly proud of our regional Neurosciences Centre; Radiotherapy Centre; Hyper Acute Stroke Unit; and dedicated breast care service at King George Hospital. We're also pleased to be part of the NEL Cancer Alliance.
