Job summary

Come and join our Cheshire and Merseyside Rehabilitation Network; a collaborative pathway delivered across multiple organisations from hyper acute to community services, providing specialist rehabilitation to patients who have suffered traumatic injury or illness dependent on need and not diagnosis.

We have an exciting opportunity for a Service Lead who will operationally manage a busy Rehabilitation Coordination Service, leading and managing a clinical and administration team to effectively perform key functions of rehabilitation coordination, single point of contact and bed management.

You will play a key role as Single Point of Contact for referrals into the network, providing comprehensive high quality clinical assessment of the specialist rehabilitation needs of the individual, supported by clinical criteria and with support from Rehabilitation Consultant Team where needed. As first point of contact you will be required to liaise and communicate clinical information with referrers and the specialist rehabilitation multi-disciplinary team, supporting timely admission as appropriate.

Main duties of the job

To ensure the Rehabilitation Coordination Service operates in a safe, effective and efficient way as part of the whole network.

Facilitate pro-active links and collaborative relationships between the Service & the wider system to support clinically appropriate and timely referrals.

Ensure that effective communications systems are established, maintained and monitored regularly within the Service, between teams & across referring services/organisations.

Ensure appropriate and timely transfer of care across network services.

Ensure that the Service maintains an accurate database.

Provide line management, including management of performance, for staff within the Service.

Ensure arrangements are in place for all staff members to receive regular supervision, appraisal and have their personal development plans and training needs identified and where possible met.

Responsible for service performance including monitoring of the delivery of targets and reporting as appropriate, meeting internal and external information requirements and data quality.

Lead & motivate staff to provide a pro-active and responsive high-quality service, contributing to ongoing audit, service improvement and evaluation.

Work closely with the CMRN Manager and Clinical Lead to agree clear & deliverable objectives & to ensure they are implemented & monitored.

Represent the Service at key meetings.
