Network services are offering an exciting opportunity for a Cancer Support Worker to join their team at Clatterbridge Liverpool to work with the breast MDT to lead the implementation of a new clinical model for review of CDK4 inhibitors.
It is an exciting time to be involved with this team where services are always being developed, to improve the management pathway of patients in this group.
The successful applicant must be motivated and enthusiastic about the specialist care needs of this patient group. They should be an outstanding communicator, able to demonstrate first-rate organisational and multitasking skills and have the ability to prioritise effectively and efficiently in a busy and fast paced work environment. You should possess advanced computer skills and a knowledge of community health services. They should be able to work well autonomously and as part of a strong multidisciplinary team in order to deliver and maintain a high quality of care.
Main duties of the job
The successful individual will have a role in contributing to the delivery of the Trust Objectives as well as objectives for the Breast Site Reference Group ensuring that the care delivered reflects the trusts vision and values.
The post holder will conduct holistic needs assessment and support to patients and their carer`s throughout their care pathway, as well as providing support to staff throughout the trust. The post holder will support the consultant and nursing team as well as signposting services for patients
