Job summary

Supporting people with learning disabilities and or autism while attending the acute hospital, either as an inpatient or for appointment's, making sure they always receive the appropriate care and support. Advising on any reasonable adjustments that may be need and putting these in place when appropriate.

You will need to have skills to make clinical decisions and to promote self-management. You must develop effective and collaborative working with healthcare and multi-agency colleagues and includes identifying and meeting the training needs of staff as well as providing specialist and nursing advice.

You will be responsible for managing the interface between mainstream general hospital services, primary care services and community specialist learning disability services in supporting people with learning disabilities when requiring hospital services.

Developing new and exciting ways of working and sharing skills within the acute settings.

Main duties of the job

This post has been developed to increase access and reduce health inequalities for people with learning disabilities and/or who are on the autism spectrum.

Act as a resource for the acute hospital regarding the complex health needs of people with a learning disability and or on the autism spectrum.

Manage the interface between mainstream general hospital services, primary care services and community specialist disability services.

Challenge professional and organisational boundaries and barriers, identifying areas for skill/knowledge development to ensure that people with learning disabilities and or autism spectrum conditions have equity of access to healthcare services.

Develop effective and collaborative working with healthcare and multi-agency colleagues, including identifying and meeting the training needs and providing specialist and nursing advice.

Incorporate the philosophy outlined in "Valuing People", working closely with the Equality and Diversity, Inclusion lead.

Work in adherence with the Mental Capacity Act 2005, advising other staff on its correct application and working closely with the Mental Capacity Act Lead.

Work on complex patient cases, for elective procedures, liaising with all professionals around the patients to co-ordinate admissions and all reasonable adjustments that are needed to be put in place.

Providing training and resources for the RCHT staff in line with government advice and legislation.
