Job summary
Royal Cornwall Hospital is looking to recruit a stoma care nurse to join their established team of stoma care nurses.
The Stoma Care team is based at the Royal Cornwall hospital but see paients in all community hospitals and in their own home.
The stoma care department at Royal cornwall hospital sees around 300 new patients a year.

Main duties of the job
The post holder will provide long term management to patients with a stoma.
This specific care will be provided through comprehensive clinical skills, information advice and support to patient before stoma formation durring there hospital stay and post discharge.
It will invove seeing inpatients at the Royal cornwall hospital as well as other community hospitals. Nurse led clinics at different setting throughout Cornwall and Isle of Scilly. It will also involve seeing patient in their own home.

Manage own case load, undertaking nurse led clinics and providing expert advice and guidance, information and effective decision making so that patients and their families receive the appropriate information and education about stoma management.
Understand and recognise own limitations and refer to a more specialist nurse when required.
Act as a patient advocate when assessing and interpreting information to enable patient involvement in decision making around treatments options and in making informed choice.
Provide optimum liaison and co-ordanination of care to patients between primary and secondary care including pre- operative assessment and discharge planning.
Identify patients physical, social spiritual and psychological needs in order to refer as appropriate to other agencies, utilising both professional and voluntary resources at both local and national level.
Act as a resource on the management of pain, wound care and bowel management in conjunction with other specialists.
Assess clinical and social needs and ensure the provision of choice of stoma appliances.
Maintain accurate and concise records.
