Targeted lung Health Check lead Nurse
An exciting opportunity has arisen within the Respiratory Team to support the development of our Targeted lung health check service.
The purpose of the role is to facilitate the clinical flow of the Targeted Lung Health Check (TLHC) lung screening participants that have an abnormality reported on their CT scan and support the delivery of the national optimal lung cancer pathway within Cornwall, both for screening identified patients and those referred via conventional routes. The successful candidate will act as a conduit between the external supplier's nurses, local medical teams across both primary and secondary care, and screening participants.
Main duties of the job
Working closely with key stakeholders this role will be central to the delivery and successful implementation of the screening programme and the optimal pathway. The lead nurse will have clinical oversight of patients on the screening pathway and be responsible for facilitating onward referrals as recommended at the Screening Review Meeting (SRM); the role will be supported by a TLHC Navigator and the Responsible Assessor/Lead nurse for the programme.
This is a pivotal role to the programme; the Lead Nurse will contribute to preparation of the SRM ensuring past medical history or scans are noted and where applicable patient called to prepare them for onward referrals or additional investigation. This role requires working closely within the Respiratory Team, Lead Clinician and Radiologists to facilitate a robust process which ensure streamline pathways for patients whilst controlling the burden of onward referrals and diagnostics on secondary and primary care teams.
The post holder will also work alongside the Lung Cancer Clinical Nurse Specialists in their role of providing a service for the care of and ongoing management of patients within Cornwall suffering from lung cancer, mesothelioma, or thymus cancer, working autonomously and independently within an integrated collaborative team.
