Job summary
An exciting opportunity has arisen for a dietitian to join our experienced teams of diabetes dietitians and renal dietitians. The focus for this post will be to work as a valued member of the multidisciplinary diabetes team and the multidisciplinary renal team, to provide excellent specialist care for adults with diabetes and adults with renal conditions, within a supportive and forward looking wider dietetic department.
This post would suit a motivated, enthusiastic dietitian who is looking to advance their career within the specialist fields of diabetes and renal or a Band 6 dietitian looking to work in new specialities. The role will involve the provision of structured diabetes education programmes as well as one-to-one consultations and the engagement of multi-professional colleagues in the implementation of practices.
This post is based at the Cornwall Diabetes and Endocrine Centre, Royal Cornwall Hospital, Truro and the Therapy Department, Royal Cornwall Hospital and will involve a mix of remote and face to face group education as well as outpatient clinics at various sites.
There is also the possibility of further fixed term hours working with Primary Care Networks to support improving diabetes education and care in the wider community.
Main duties of the job
Working as part of the diabetes dietetic and renal dietetic teams, to advise and educate adults with diabetes and/or renal conditions, their carers and healthcare professionals with regard to the appropriate dietary management of their condition; employing behaviour management skills as required.
Delivering dietetic outpatient clinics, including face to face and remote consultations (telephone / video appointments) for adults with diabetes, as well as adults with renal conditions (to include pre-dialysis and haemodialysis clinics). To potentially deliver dietetic care on the acute renal wards, once established in the role.
Delivering Structured Education Programmes for adults with Type 1 Diabetes and / or Renal Disease.
Working closely with and as a integral member of the the wider multi disciplinary diabetes and renal teams (including Diabetes Physicians, Renal Physicians, Physician Associates, Diabetes Specialist, Renal Specialist Nurses and Diabetes Educators), to provide evidence based and high quality diabetes and renal care.
Advising and teaching medical, nursing, healthcare, fellow AHP staff and students on the dietary and lifestyle management of diabetes and also renal conditions.
Furthering best practice by making use of current clinical evidence in the development of departmental guidelines, patient literature, policies, protocols and standards of best practice.

To work as part of a team of 1.14 WTE diabetes specialist dietitians and 2.1 WTE renal specialist dietitians and as an integral member of both the multidisciplinary diabetes and renal teams so as to ensure a holistic approach to the care of adults with diabetes and adults with renal conditions in Cornwall. This will involve regular liaison with Diabetes Physicians, Renal Physicians, Physician Associates, Diabetes Specialist Nurses and Renal Specialist Nurses.
You will be required to organise your own workload and to be adept in prioritising and balancing patient-related and professional demands and contributing to the effectiveness of both the diabetes team and the renal team.
To assess patients' current diagnoses, past medical history, relevant medical treatment and biochemistry to enable a full and thorough consultation regarding dietary management of their diabetes and /or their renal condition.
To obtain and analyse relevant information from a variety of different sources including: the patient and/or carers, electronic medical records and GP Referrals in order to make a comprehensive assessment of the patients condition and treatment plans so that a decision on the most appropriate dietetic treatment to support diabetes and / or renal management can be formulated.
To evaluate the effectiveness of dietetic goals and modify these goals and care plans as necessary based on ongoing analysis and assessment of the information available and changing patient needs.
To keep a portfolio outlining evidence of continual professional development(CPD). This may include critical evaluation of scientific papers, journal clubs, attendance at professional development meetings/remote meetings, conferences, working as a reflective practitioner and attending relevant internal/external courses/training as required.
To participate in annual appraisal, identifying own development and training needs and setting personal development objectives with support from peers.
Actively participate in the dietetic student training program, facilitating the development of skills and knowledge, being involved in the assessment process and acting as a mentor as required.
To be an active member of the wider dietetic department and taking responsibility for projects to enhance dietetic development, as required
To provide dietetic support for other specialities as required.
