We have new, exciting Transitional Care Senior Neonatal Staff Nurse/Specialist Midwife Band 6 opportunities within our 25 bed/cot Transitional Care unit, you will work as part of the Transitional Care team supporting families to care for their babies requiring more than normal care requirements but do not need to be admitted to the Neonatal Unit. This role will act as the link between Delivery suite, the Neonatal unit, Transitional care, and the Neonatal Community Outreach team. This is required to promote safe, early discharge of babies from Transitional care and prevent parent/infant separation and avoidable admission to the Neonatal Unit.

The prime focus of family integrated care is to keep babies and families together in order to enhance parent/infant bonding, reduce length of hospital stay and readmission rates all leading to improved chances of a healthier future for the whole family.

Full and part time posts are available working a minimum of 23 hours each week.

Main duties of the job

To provide clinical leadership to the nursing and midwifery team and ensure the provision of high quality nursing care to infants and their families.

To act as a role model and expert clinical practitioner.

Act as the experienced T.C. Neonatal nursing lead on duty day or night providing a constant link between Medical, Midwifery and Nursing staff on the Neonatal unit, Transitional Care and Delivery Suite.

Ensure safe provision of care through correct allocation of /Neonatal nursery nurses to T.C families.

To facilitate the implementation and evaluation of care pathways within T.C.

To promote a progressive attitude to the continual improvement of patient care through research and evidence-based practice.

To communicate effectively at all levels within the multi-disciplinary team.

Direct point of contact for all staff to provide a clear escalation pathway to the Neonatal medical team.

CLINICAL:

Identifies a deteriorating baby and takes the appropriate action to manage the situation, liaising with the multidisciplinary team as appropriate.

To ensure the provision of excellent, evidence based care for infants and their families.

Offer professional advice and support to families whose babies require Transitional Care (TC)

Work in partnership with parents/carers to support and guide them in the care of their newborn infant promoting family centred and integrated care.

To enable parents to become partners in care for their babies. Supporting them with informed decision making processes and entrusting them with all possible care.

To be a skilled supporter of infant feeding, providing excellent breastfeeding, expressing and responsive bottle feeding support to families.

To act as an advocate for the baby and family, providing them with information and support and directing them to the relevant support services where appropriate.

To start planning discharges from admission, liaising with the multidisciplinary team, and community Neonatal Outreach services as appropriate.

To maintain clinical skills and knowledge necessary to provide holistic, evidence-based nursing care. This includes expanded role responsibilities, following appropriate training.

Assist with Neonatal education opportunities for Transitional Care staff as an experienced Neonatal Nurse working together with Neonatal clinical skills facilitator.

Communicate effectively, promoting open and trusting relationships.

To be aware of the uses, safety precautions and handling of equipment in the department.

Work with members of the team, in the development and review of policies and standards for the safe use of equipment and any new equipment introduced to the department.

To support and facilitate others in completing the neonatal competency packs.

MANAGERIAL:

To lead, supervise and support members of the nursing/midwifery team and ensure that all babies receive safe and appropriate care.

To lead and support initiatives towards achieving and sustaining optimum care within the TC service in line with the BAPM Neonatal T.C Framework for Practice.

Maintain up-to-date procedures, policies, and guidelines applicable to the service and to support in writing TC reports as required.

Assist in supporting Neonatal and Maternity leads to minimise term admissions to the Neonatal unit, helping to identify themes and trends which should inform improvement projects.

To document TC Care in the babys clinical records to ensure that contextual assessment is ongoing, and to ensure improvements in communication with MDT.

To liaise with administrative staff to ensure TC Babies admission episodes are entered on Badger Net to ensure TC activity is present and accurate on quarterly reports and future planning.

Be involved in developing TC care pathways to ensure babies identified as requiring TC care can receive care based on acceptable standards.

To maintain an active role in clinical governance initiatives.

To assist the investigation of incidents, reported via the Trust Datix systems, related to TC.

Support the provision of effective communication between all members of the multidisciplinary team, other hospital departments and TC families.

Manage allocated resources effectively and efficiently. This includes maintaining the correct skill mix of staff for patient dependency.

To ensure the safety of the department, and the risk management process by actively participating in clinical audit, clinical risk and quality issues, including the initial management and reporting of incidents.

To ensure that all staff know where to access trust policies and clinical procedure guidelines and that staff always adhere to these.

Act as a team leader for Nurses/Nursery Nurses, acting as a mentor/preceptor, and facilitating their development through challenge and support.

To lead and co-ordinate team projects and team building activities.

Participate in the process of recruitment and retention of staff.

Promote and support the improving working lifes philosophy, and initiatives.

To contribute to the appraisal process and manage staff short term sickness and attendance.
