Job summary
The post holder will:-
- Work alongside duty Wren midwife to plan and support care in acute and community as required on the day
- Provide birthing people and their families with information on the parent and infant relationship to optimise parent and infant mental health
- Escalate all expressions of concern made by birthing people to Wren midwife or appropriate manager
- Provide support and joint working alongside Community midwives and MSWs - overseen by Wren midwife
- Support with breastfeeding, core parenting, caring for baby etc taking pressure off midwives on the wards and ensuring that those with the greatest need receive additional support and continuity of carer where possible
- Provide support as required on wards and as agreed with duty Wren midwife
- Deliver bespoke parenting sessions in the home for those with complex needs and support other MSWs with core parenting as required
- Attend home visits alongside the midwife, especially when lone visiting not recommended
- Daily huddle with safeguarding team
- Communicate with PMH women and hold a small caseload overseen by PMH midwife
Main duties of the job

- Consistently employs strategies to promote personalised, informed decision-making when planning, implementing and evaluating care activities. Tailors information to meet the needs of the birthing person, including where complex care and support needs exist

- Consistently applies the principles of continuity of carer when making judgments related to supporting service users with complex needs
- As a key member of the MDT, arranges access to services to support personalised care plans signposting and referring service users and their families to agencies and services appropriately
