We aim to support out patients to dialyse where,when and how they choose.We are now able to offer a new oppurtunity to work with our renal clinical nurse specialists to support people to dialyse at home.This is a new post that will give the successful candidiate a real chance to make a real difference. Along the way you will need to learn new skills which are renal specific, whilst also developing the skills you bring with you.
This is a 37.5 hours per week role. You will be required to work flexibly with occaisional weekend working. There will be lone working and access to a care and the ability to drive is crusial as you will be visiting patients in the community.
Main duties of the job
The Nursing Associate has a breath of knowledge across the lifespan and across fields of nursing, providing holistic and person centred care, and support people of all ages in a variety of settings. Nursing Associates are equipped with the knowledge, understanding, skills, attitudes and behaviours relevant to their area of employment and will work to the NMC Code of Conduct.

Work as a member of the multi professional team providing clinical care to patients in the home and community settings as delegated by the registered nurse or registered health care professional.
Ensure a high standard of patient-centred care.
Promote self-management of health needs and adopt a positive risk based care approach.
Key working relationships with all members of the multidisciplinary team, including nursing and medical staff, allied health professionals, support workers and students.
Will have responsibility to deliver nursing care to patients with chronic kidney disease within their own homes or in the acute hospital setting. This may involve supporting the delivery of peritoneal dialysis and home haemodialysis and involve lone working. Follow individual care plans as described by the multidisciplinary team within own scope of practice and competence.
