Applications
are invited for a post as consultant in respiratory medicine. This is a
replacement post joining a team of 13 consultant currently providing elective
and non-elective services from two acute hospital sites in South Tyneside and
Sunderland. The team are supported by specialist nurses, and provide care for
the local population of nearly half a million people, with a high prevalence of
acute and chronic respiratory disease.
Candidates with
general respiratory experience are invited to apply, and various sub-speciality
interests can be accommodated.
Main duties of the job
This is an
exciting time to be joining the Respiratory Team at South Tyneside and
Sunderland with our continued service expansion. The successful applicant will
be encouraged to develop an area of sub-specialty interest as well as
supporting the existing services as appropriate across both sites.
A Hospital Consultant has continuous
responsibility for any patient under his or her care.
Final job plans will be agreed in
discussion between the successful candidates and the Clinical Director and will
be subject to Annual Review.
Some sessions, in particular audit
and teaching sessions will be worked flexibly and there will be the opportunity
to revise the fixed procedural sessional timetable to accommodate other
commitments of the appointee.

At South Tyneside, you will have
continuing responsibility for care of your in-patients on Respiratory Medicine
(Ward 10 and outliers) and your respiratory outpatients; you will cross-cover
in-patients of Consultant colleagues during periods of holidays, study leave
and (short term) sickness. You will be expected to supervise junior Medical Staff
allocated to you and you may be a nominated educational and clinical supervisor
to identified junior medical staff assigned to Respiratory with appropriate
training provided if necessary. If you are an Educational Supervisor, this will
attract additional PAs in your job plan to cover for this. You will also be
expected to undertake as part of the team an active role in regular
departmental Clinical Governance meeting, including morbidity & mortality
(M&M) reviews.
You
will be expected to undertake ward rounds twice a week, board rounds on
non-ward round days and administrative duties directly associated with the care
of patients and the running of clinics.
This includes the SIMS checklist at the start and end of your
allocated day. This is a rapid check
done with the ward juniors, to identify patients requiring immediate review,
procedures to be performed/supervised, help co-ordinate the ward juniors in
relation to responsibilities for that shift, and to highlight tasks/patients to
be handed over to the on-call team.
You
will share responsibility for provision and development of the respiratory
service with the Consultant team and will work closely with the Acute
Respiratory and Lung Cancer Specialist Nurses and Respiratory Physiology staff.
There
will be a commitment toward General/Acute Medicine, depending on your
interests; this can take the form of being the Consultant on-call for General
Internal Medicine (GIM) as per the GIM rota, or being the daytime Consultant
Acute Physician (CAP) based on the CAP rota.
Both are based on the Emergency Assessment Unit (EAU) at South Tyneside
which receives all medical admissions to the hospital. Further details can be provided on request.
Presently,
the GIM Consultant takes a handover from the daytime CAP between 5 5.30pm.
Daily admissions range from 30 50 patients per day.
Currently
the Consultant team at the Sunderland site operate a 6-7 week rotation. At any
time there are 2 consultants involved in supervision of respiratory inpatient
care.
Week-1
(Monday-Thursday): The Consultant on-call is responsible for new admissions to
respiratory and continuing care for inpatients admitted more than 7 days
previously. There are no clinics or other elective work during week-1.
Responsibilities are: Supervision of care for respiratory in-patients;
provision of respiratory opinion to other departments. At weekends (Saturday
& Sunday) the duty consultant will undertake a ward round of respiratory
in-patients: review new admissions; acutely ill patients, patients with chest
drains or those on NIV; identify patients for discharge.
In-patient
pleural procedures are usually performed by the respiratory registrars
depending upon experience. Consultant supervision is occasionally required.
Week-2:
Consultant is responsible for patients admitted under their care during week-1
and will be on call from Friday until Monday am and has a limited number of
elective duties.
Weeks
3-6: Elective activities only, including outpatient clinics and
bronchoscopy/EBUS. Clinics include new and review patients (template
negotiable); protected slots for 2 week-wait (suspected cancer) referrals.
The
Respiratory Consultants participate in the acute medicine on-call during the
on-call week (week-1). This involves 1-2 evening shifts (3-4 hours) in the
integrated assessment unit (IAU) reviewing/triaging newly admitted medical
patients.
It
is anticipated that the entire team will contribute to co-ordinated working
across both sites and improve efficiency and patient pathways. Development of
subspecialty interest would be encouraged.
