Job summary
An exciting opportunity has
arisen for an Alcohol Care Team Recovery Navigator to join the Alcohol Care
Team (ACT) at STSFT on a fixed term 12 month contract. Any
secondment would require discussion and agreement from your current line
manager.
ACTs are teams of alcohol specialist clinicians, based
in acute hospitals which provide specialist support, predominantly to
alcohol-dependent patients.
The Alcohol
Care Team Recovery Navigator will be an integral member of the ACT working
within the team to support patients in their recovery journey to access support
of their choice, the post holder will work with vulnerable patients completing
assessments, extended brief intervention, referrals and links with community
services. The post holder will work with Navigators already in post within
ACT.
The post will require flexibility, working at Sunderland Royal Hospital,
you will also contribute to covering a 7 day service. The successful candidate will be able to
demonstrate excellent communication skills, team working skills and will be
dedicated to improving the patients' experience. You will need to demonstrate
continuing professional development and a strong commitment to develop others
and work closely within a multi-disciplinary team.

Main duties of the job
The Alcohol

working. The successful candidate will

will be dedicated to improving the patients' experience.

The Recovery/Social Navigator will

users with alcohol misuse issues in a hospital environment.
The post holder will deliver and

improve the outcomes and life chances for adults experiencing multiple problems
including drugs and alcohol dependency and reduce the impact upon emergency
services and local communities
Form strong professional
relationships with the emergency department team and work closely with them to
identify patients who may benefit from the Navigator service.
Responsible for engaging new
individuals onto the service by working closely in partnership with treatment
and recovery providers and with the recovery community
Support service users through
community outreach; build strong trusting and respectful relationships that
enable them to regain independence and control over their decisions.
Connect service users to existing
community support (of their choice) and support as necessary to maximise
engagement.
Hold a case-load and respect service
user confidentiality and adhere to GDPR principles
Assist with the on-going development
of the service model to ensure that service users gain the maximum benefit from
Navigator interventions.
Provide a consistent support role to
the individual to navigate through all the various services but ultimately advocating
for the individual where they need to, and to support the individual to become
independent too.
Ensure the service is well known,
visible and accessible to potential beneficiaries.
Undertake sign posting and referrals
to all community and voluntary support services.
Ensure administrative systems such
as service user forms, databases and contact files are kept fully up-to-date
and secure, in line with the Data Protection Act and our Confidentiality Policy
and Consent Policy.
Collect and record evidence of
project outcomes, to assist with the evaluation of Navigator.
Continually develop personal level
of awareness and knowledge of the issues around all forms of violence and
causal factors.
Actively research and network with
other relevant organisations working in the local area. To build personal and strong partnerships
with them, ensuring Navigator is able to offer as diverse sign posting options
as possible, such as housing support, welfare support, social care etc. so we
are addressing those social determinants of health which often are the basics
to put in place before we can then address complex misuse issues.
Actively participate in staff
meetings, session evaluations, supervision/reviews and planning sessions, and
to participate in Navigators development by feeding back to Line manager and
governance groups on the needs of the service user group and the on-going
development of the service.
Assist in maintaining good working
relations with Navigators stakeholders, project partners and with other
agencies and to represent Recovery/Social Navigators at external meetings and
in the media as required.
With guidance and support from the
organisation undertake continuous professional development, through attendance
at courses and other educational activities, relevant to the role.
To work as a member of the wider
team and assist as required across all the organisations projects (this will
be minimal).
Engage and recruit Navigator
volunteers whom may have been previous service users who have recovered and are
able to offer peer support for new clients.
To assist the team in ensuring that
all Navigator and NHS policies and procedures are followed, including Health
and Safety, Child Protection and Equal Opportunities. To report any concerns to
the Line Manager.

of individual patients who are not engaging in the community but continue to
