Young Peoples Diabetes Service within the South Tyneside and Sunderland NHS
Foundation Trust. We are seeking a person who is enthusiastic, dynamic and
committed to delivering high quality paediatric diabetes care.
We are seeking
someone who is passionate about paediatric diabetes and achieving optimum
health outcomes for all children, young people and their families living with
this long term condition.
This post is a
band 6 PDSN.
Service
performance and outcomes are continuously being measured against NPDA, peer
review measures and best practice tariff
criteria. We fully participate in the National Paediatric Diabetes Audit and
are able to benchmark ourselves against similar services. We are active
participants in the regional paediatric diabetes network and the RCPCH
Paediatric Diabetes Quality Programme. We are enthusiastic adopters of all
diabetes technologies and have an active, effective QI programme. For many years, metabolic outcomes for our
children and young people have been amongst the best in the country.

Main duties of the job
- The successful candidate will be part of the South Tyneside and Sunderland Children and Young Peoples Diabetes Service and will be part of an MDT which operates from two base sites in Sunderland and South Tyneside. The wider service includes a total of 5.5 WTE PDSNs and 1.0 WTE HCA delivering care to approximately 280 CYP with diabetes. This role will initially be predominantly based at Sunderland Royal Hospital but flexible working across both sites may be needed.
- To coordinate the delivery and development of the specialist CYP diabetes service through the use of effective evidence based practice in line with agreed national standards for patient safety and monitoring
- To advise, support, train and supervise the HCA in the MDT
The post-holder will hold a high degree of autonomy and responsibility for assessing, developing and delivering care pathways for children in liaison with the 5 diabetes consultants and the MDT.

governance framework.

exercise accountability as set out in the NMC Code
To ensure
ongoing relevant continuing professional development in the field of paediatric
