Job summary

Would you like to work in a relatively new role within the Trust providing support to both inpatients and maternity around Tobacco Dependence? Funded through the NHS Ten Year Plan to provide tobacco dependence support while in hospital and pregnancy. This is a12-month temporary opportunity. We are looking for candidates who have an NVQ level 3 or equivalent qualification, excellent communication skills and an empathetic manner, an ability to work under pressure as well as the ability to deliver training, some knowledge around tobacco dependence and treatment and able to drive to community venues. Main duties of the job

To provide behavioural support and treatment in relation to tobacco smoking.

Provide training to employees on screening and brief interventions.

To keep timely and accurate records of interventions and outcome activity and use this in decision making, using the Trusts policies, procedures and electronic recording and data collection system.

To signpost into other services and integrate care with other providers of health services.

To comply with the Trusts vision, values and behavioural compact.

The service will operate during office hours with Saturday working on a rota basis.

To work with the onsite tobacco treatment service and specialist nurse to provide behavioural support and treatment in relation to tobacco smoking.

To support the day to day running of the onsite tobacco treatment service to ensure safe, well organized and evidenced based care is offered routinely to all identified smokers and provided to all that access the service. Provide support in a range of settings as required for the delivery of smoking cessation service.

To support the delivery of the service key performance and reporting indicators for tobacco dependency.

Provide training to employees on screening and brief interventions and support the Team in developing and delivering education to Trust employees in the speciality of tobacco dependence.

Maintain a high standard of work and prioritise own workload according to the needs of the service, individual needs including equality, diversity and social inclusion in all aspects of work.

Take referrals, including self-referrals into the service and work with patients to identify the most appropriate tobacco dependency treatment pathway.

Identify and engage with individuals and groups from target areas.

Support individuals to make informed choices about their health and wellbeing and support them in initiating and sustaining appropriate behavioural changes to improve their health.

Record and report activities and results.

Develop and sustain tobacco dependency support and referrals into the service.

The tobacco dependency service will operate seven days each week.
