Job summary

We are seeking to recruit a full-time enthusiastic health care assistant to work in the Community Planned Care Teams based at Clarendon, Hebburn.

The post-holder will be required to provide nursing care to clients in Health Centres, G.P. Surgeries and in clients own homes over a seven-day period including bank holidays.

Must have recent experience working in a nursing environment, educated to NVQ level 2 / Care Certificate and have a driving licence with access to a car.

Main duties of the job

Completing nursing tasks delegated by a District Nurse e.g. simple wounds, administration of insulin, in patients own homes, residential homes, clinics.

Supporting all staff within the District Nursing Teams of ST.

To work within the Integrated Care Team, providing care to patients in their own homes / care home / other care settings providing assessment and treatment of patients in the community over a 8am-8pm period 7 days a week. It will also support appropriate early discharge of patients and will give support to Primary Care and the Unplanned Care Team.

To carry out planned and unplanned health (nursing) interventions as designated by the lead clinician, and will work both supervised and unsupervised in patients own homes, residential, nursing and other intermediate care facilities, reporting back patient progress and informing the lead clinician of any problems.

The postholder will work flexibly across boundaries of care to support patients in their own homes based on identified need.
